Forms and fees

Access forms and fees of the New Zealand Companies Office. You will find the full range of forms for New Zealand and overseas companies. There are also forms for other registered entities.

Forms for New Zealand companies

Almost all your business with the New Zealand Companies Office can now be completed online. To use any of our online services, select the service you want from the Online Services tab above.

Australian overseas company forms

These forms are for Australian companies carrying on business in New Zealand, as defined by the Companies Act 1993. There are online services available to apply to reserve a company name and register as an overseas company. However, to maintain details for Australian overseas companies you need to complete and submit paper forms.

Other overseas company forms

These forms are for overseas companies (other than Australian companies) that are carrying on business in New Zealand, as defined by the Companies Act 1993. To register or maintain details for overseas companies you need to complete and submit paper forms. The one exception is that you can apply to reserve a company name online.

Forms for other registered entities

The Companies Office maintains a range of other registers in addition to the Register of Companies. Here are forms for Retirement Villages along with Annual Return forms for Credit Unions, Friendly Societies and United Friendly Societies Dispensary and Working Mens Clubs. Financial Reporting Act

Attach a Fast Track Filing Form to all financial statements that you file under the Financial Reporting Act 1993. Complete the checklist on the form to individually tick off all items required to be included in the financial statements you are submitting.

Direct debit form

Use this form to advise your bank account details for direct debit payments made using your User ID. If you set up or alter the direct debit details online, we will email you a direct debit form that is pre-filled with the details you supplied. The signed form must be returned by mail to our Revenue Unit for processing.
