Forms for New Zealand companies

It is now mandatory to complete your company administration online

Mandatory online filing was introduced on 1 July 2008 and has seen the removal of many of the manual processes that the Companies Office previously supported.  Name reservations, company incorporations, annual returns and general company maintenance such as the appointment of directors must now be completed online.

To use any of our online services, select the service you want from the Online Services tab above.

 

Exceptions

There are some situations where an online service option is not available and these are outlined below.

If you are unsure whether you are able to file a paper form for a particular situation please Contact Us and check.  If you file a paper form and there is an online service available, your paper forms will be rejected.

 

Notes about our PDF forms 

To view or print our forms you will require PDF viewing software such as Adobe Acrobat Reader.  If you are experiencing problems downloading our forms, make sure you have updated your PDF reader to the latest version.

While viewing the form in your PDF viewer you can enter details directly into fields on these forms before printing them.  Alternatively you can print a blank form and complete it neatly by hand.

Note | This set contains the application and consent forms (forms 1, 4 and 13).

You will only need to use form 4 where the name you want to reserve is that of one of the companies due for removal on amalgamation.  Otherwise use the online service.

Preparing director's or shareholder's consents for a new company.Personalised consent forms will be sent to you at the time you register a company online. 

Whether you use the personalised consents or you enter the details yourself into these blank forms, you must fax or upload the signed consents to complete your application for registration.

Preparing accounts of a company in voluntary administrationDownload form VA1 - File accounts of a company in administration [96 kB PDF]

Use this form to supply the details prescribed in Schedule 2 Companies (Voluntary Administration) Regulations 2007, for administrator's accounts. 

An electronic version of the completed accounts can then be uploaded using the Voluntary Administration Online Management service.
